Match the exact plan or network name on your ID card. When in doubt, it can be helpful to call the provider's office directly and verify your plan is in-network.
In-network doctors have agreed to your insurance’s pricing, so you’ll pay less.
Out-of-network doctors haven’t, meaning your costs could be much higher (or not covered at all).
It depends on your plan. HMO plans often require a PCP referral to see a specialist. PPO and EPO plans typically do not require referrals in order to see a specialist.
